Vital Statistics Division

Due to updates being completed by the Ohio Department of Health (ODH), the Vital Statistics office will not be able to issue birth or death certificates on Tuesday, August 21, 2018 during the updates.  ODH estimates that the updates will take place approximately 7:00am to 11:00am on this day.  It may run longer, but this is a general estimate of the time.

Canton City Public Health has records for births that occurred in the State of Ohio and for deaths that occurred within the City of Canton limits only.  The cost of each certificate is $25.00 per copy.

Corrective affidavits for birth records as well as declaration of paternity forms, are prepared at no charge.

Walk-In Service
Birth or death certificates may be purchased Monday through Friday from 8:00 am to 4:30 pm at 420 Market Avenue North.  No identification is required.  We accept cash, checks, Discover, Visa, MasterCard, debit card or money orders.

Telephone Ordering
Contact the Health Department at (330) 489-3231 Monday through Friday from 8:00 am to 4:15 pm to order a birth or death certificate over the telephone.  We accept Discover, Visa, MasterCard or debit cards.  Your certificate will go out in the mail the same day you call in to order.

Click here for the Birth/Death Certificate Request Form.  Mailing instructions are listed on the form.  Most requests by mail are answered within two business days after the receipt of the written request.  Payments must be a personal check or money order.  Please include a telephone number on all checks.
