Referral for Community Health Worker

What is a Community Health Worker?
Community Health Workers (CHWs) are frontline public health workers who are from and live in our community. Certified by the Ohio Board of Nursing, CHWs serve as a link for connecting individuals and families to facilitate access to medical, social, and behavioral services at NO COST to improve infant vitality and promote quality of life.

If you are pregnant or have a child under age 1, and would like more information about how a Community Health Worker can help you, fill out the online Request for Community Health Worker Referral Form.

You will be contacted within 48 hours by a Community Health Worker who will explain the program in full and get you enrolled. Every baby deserves to celebrate their first birthday, let us help you and your baby be healthy!
